MULTI-AGENCY TRANSITION MEETING (MATM)

Background

In 2019, MOE, in partnership with MSF, initiated a collaborative project, the Multi-agency Transition Meeting (MATM), to strengthen the handover process from SPED schools to Adult Centres, promoting more person-centred planning for the student to enhance quality of life outcomes.

What is MATM? 

MATM takes the form of a multi-party meeting between the key stakeholders from school and post-school adult services such as DACs and SWs to support the student’s transition to the Adult Centre.
